Important Policies
- Deposit Required: A deposit will be fully refunded upon visit, applied to co-pay, or refunded with more than 48 hours' notice.
- No-Show Policy: If the appointment is a no-show without prior cancellation notice, an additional $150 will be charged to the credit card on file.
- Same-Day Cancellation: If the appointment is canceled on the same day, an additional $75 will be charged to the credit card on file.
- Canceling/Rescheduling: Save and use the link in your appointment confirmation email.
- We do not prescribe benzodiazepine medications.
- We may not prescribe stimulant medications for ADHD on the first visit, even if previously prescribed by another provider.

Insurance Information
- Not Accepted: HMO plans, Medicaid, Medicare.
- Primary Insurance: BCBS PPO must be your primary insurance.

Updating Your Information
- Established Patients: Notify us immediately of any changes in insurance coverage or personal legal info (like a new name or address).
- Reminder: Failure to update us before your appointment may result in out-of-pocket charges.
